  2. Dates will be checked for availability
  3. Devone Polen will contact applicant IF adjustments need to be made.
  4. Verifications will be mailed to the applicant.

As a representative of the applicant, the undersigned agrees to the use of the Miley Community Center in accordance with the policies of the Clark Community Presbyterian Church and conditions of rental/usage as stated in the attached. The applicant agrees to indemnify and HOLD HARMLESS Clark Community Presbyterian Church and their agents from all liability, claims, demands, damages, or costs, for, or arising out of the usage caused by the negligence or actions (whether intentional or otherwise) of the applicant or the applicant’s member or event members.
